Analyzing the implementation outcomes of this ICM project through internal stakeholders serves multiple compelling purposes. Firstly, it allows the lab to evaluate the effectiveness and success of the intervention specifically tailored to this community. By monitoring outcomes such as reach, fidelity, and sustainability, researchers can gain valuable insights into the extent to which the implementation is happening and whether our desired objectives are being achieved within the target population. These insights are instrumental in comprehending the overall impact of the program and identifying specific areas that may require enhancements for better outcomes in supporting Black Canadians. Secondly, tracking implementation outcomes enables researchers to identify influential factors that can determine potential success or failure of the intervention being examined. Lastly, tracking implementation outcomes generates evidence-based data that informs decision-making and resource allocation. This empowers stakeholders to make informed choices regarding the continuation, expansion, or modification of the intervention, based on objective measurements of achievement specifically related to the ICM project.
Research Goal
The goal of this implementation research project is to support the development of an Implementation Research Logic Model around the creation of an Intensive Case Management model (ICM) and develop a protocol in line with the logic model and pilot the tool instruments arising from this logic model. These activities will also support the refinement of the model of care for future scale-up and replication of the Intensive Case Management Programs.

Ultimately, tracking implementation outcomes is crucial for the sustainment of evidence-based practices, program refinement, and the maximization of the intervention’s potential impact on the intended population of Black Canadians in Toronto.
